5. NEO Baseball Club can remove you from the facility at any time due to anything that NEO Baseball Club sees fit of this action
6. Name on this form grants NEO staff member the right to take initial action in any emergency situation until medical help arrives
7. Name on this form is agreeing to pay full invoice on time, based on the information shown on the invoice
8. NEO Baseball Club can terminate or alter a rental, lesson or camp at any time due to lack of payment or anything that NEO Baseball Club sees fit of this action
9. Name on this form understands that if anything is paid in cash it cannot be disputed
10. Name on this form understands that they are fully responsible for all person(s) in attendance with them in their group
11. By submitting this form you herby waive all liability of NEO Baseball Club, will follow all points shown above, and release and discharge NEO Baseball Club & Mindset Sports LLC from any future claims and or legal action
